While Dr. Campbell does not accept all insurance plans directly, services may be reimbursable in full or in part by your health insurance or employee benefit plan. Every insurance company has different deductibles, reimbursement rates, and procedures for submitting claims. Please check your coverage carefully prior to your session. Reach out to Dr. Campbell if you are in of assistance with this process.

The following questions are suggested when you call the behavioral health number on your insurance card:

  • Do I have mental health benefits?
  • Does my insurance provide out of network reimbursement?
  • Will CPT Code 90791, 90834, and/or 90837 be reimbursed?
  • What is my out of network deductible and has it been met?
  • How many sessions per year does my health insurance cover?
  • Is approval required from my primary care physician?
  • What is the process for submitting a “superbill” for reimbursement?

Fees

  • First call (approximately 15 minutes) is free of charge.
  • $200 for an initial intake & assessment
  • $175 per 45-minute session thereafter
  • $195 per 53-minute “extended” session thereafter
  • $345 per 90-minute “intensive” session as needed

Starting on January 1, 2022, the No Surprises Act (NSA) protects uninsured or self-pay individuals from any unexpectedly high medical bills. The NSA requires that health care providers give uninsured and self-pay individuals a Good Faith Estimate (GFE) for the cost of their healthcare before the individual agrees to the service. This regulation is designed to provide transparency to you, the client, as well as protect you from surprises when it comes to your medical bills.

In the outpatient psychotherapy setting, there are a number of factors that make it challenging to provide a GFE for mental health treatment. Length of treatment and frequency depends on the individual client and goals in therapy. Additionally, what determines success in treatment is fluid. Some clients attend only until they experience a reduction in symptoms while others continue after a crisis has passed because they find they continue to grow and evolve. At times, clients who are not in crisis may begin to schedule less frequently and may continue to come in occasionally when issues arise. Ultimately, as the client, it is your decision when to stop therapy. It is strongly suggested by Dr. Campbell that if you are ready to move on from therapy you have one final session to wrap up what you have been working on together. It is not often we get to say a healthy goodbye.

Please note, insurance companies require a diagnosis for billing and reimbursement. If you are self-pay, you should know that the NSA requires that Campbell Counseling, LLC provide a diagnosis for each client so that a fee can be assessed for treatment. This diagnosis is tentative until a formal diagnosis can be completed a few sessions after beginning psychotherapy. If you choose to decline a formal diagnosis, we will not update the GFE. It is within your rights to decline a diagnosis per state and federal guidelines.​​​
Campbell Counseling, LLC recognizes that every client’s experience and needs are unique. How long you engage in therapy and how often you attend sessions will be influenced by a number of factors including but not limited to:

  • Client schedule
  • Therapist availability
  • The specific and unique needs of your treatment
  • Unforeseen circumstances
  • Personal finances

You and Dr. Campbell will continually assess the appropriate frequency of therapy. You will work together to determine when you have met your goals and are ready to discharge. A new GFE may be issued should your frequency or needs change.

According to the American Psychological Association, “on average 15 to 20 sessions are required for 50 percent of patients to recover as indicated by self-reported symptom measures. There are a growing number of specific psychological treatments of moderate duration (e.g., 12 to 16 weekly sessions) that have been scientifically shown to result in clinically significant improvements. In practice, patients and therapists sometimes prefer to continue treatment over longer periods (e.g., 20 to 30 sessions over six months), to achieve more complete symptom remission and to feel confident in the skills needed to maintain treatment gains.” 

Your GFE depends on several factors because everyone has unique counseling goals. It takes time to develop a therapeutic relationship with your therapist, identify your treatment goals, establish a plan of action, and work towards accomplishing those goals.

The GFE requires practitioners to provide an exact estimate and not a range. Out of an abundance of caution and transparency, we will only quote weekly appointments for a year (without skipping any weeks for holidays, break, vacation, unplanned events/sickness, etc.).

The above example is provided to give an idea of the financial expectations for a calendar year. The frequency and duration are dependent on your individual needs and goals

The appointments you make with Dr. Campbell are at times reserved exclusively for you. If for any reason you need to cancel, please call no later than 24 hours prior to your appointment. If you cannot cancel 24 hours in advance, you will be charged $50 for the missed appointment. If you neither cancel nor appear for your appointment, it is your responsibility to pay the full fee for the missed appointment. These charges are not billable to insurance. If you fail to attend without proper notification two scheduled appointments in a row Campbell Counseling, LLC reserves the right to collect the fee for the missed sessions and discontinue service. Scheduled sessions begin at your arranged appointment time. Your fee is based on that time period. Session fees cannot be prorated and session timeframes cannot be extended, therefore prompt arrival is in your best interest. Thank you for the consideration.
